REGISTERING

To register please complete a registration form and new patient questionnaire form which can be provided by our reception staff or via the links below.

You will be registered with the practice, not an individual GP. You may however, select a GP you would prefer to be seen by, but circumstances may not always make this possible.

You will then be able to complete the forms on your computer. When complete, print the form and bring it into the practice or send it via email to gram.bydandclinical@nhs.scot.

Online Services

If you would like to register for our range of Online Services currently in development please download our Online Services Application form and return it to the medical Practice in person, along with a valid form on identification. Once you are registered the practice will give you the information that will enable you to create a username and password.
