Non-NHS Servies

Not all services are covered under our contract with NHS and therefore will incur a charge.  Examples include –

• Insurance Reports
• Holiday Cancellation Forms
• Medicals for pre-employment, driving (HGV, PSV etc)
• Shotgun licence report

Please ask the receptionist who will be happy to advise you of the fee and arrange an appropriate appointment if required.

All fees must be paid before leaving the surgery.  Cash, cheque or electronic bank transfer only.
