CHANGING YOUR ADDRESS

1. Download the 'Change of address' form below. / Collect a 'Change of address' form from the Health Center.

2. Fill out the form with your old address & new address. Remember to include everyone at the address on the form. You only have to complete one form per house hold. 

3. Email the completed form to gram.bydandclinical@nhs.scot or hand the completed form into the health center reception.

 

PLEASE ENSURE YOUR CORRECT UP TO DATE CONTACT NUMBERS ARE INCLUDING ON THE FORM.
